The purpose of this job is to provide service coordination and assistance in accessing medical, social, educational, vocational, and other appropriate services to individuals enrolled in Home and Community Services. Conducts assessments, completes all documentation within required timeframes, develops person directed plans in coordination with clients, legally authorized representatives, and any other treatment team members. The purpose of these assessments is to determine if specialized services related to developmental disability should be provided to the resident or if the individual requesting admission or newly admitted to a nursing facility could be served in a less restrictive setting in the community, diverting them from nursing facility admission. These duties will also involve providing habilitation coordination to a caseload of residents who are residing in nursing facilities to assure that appropriate services are delivered while they remain in the setting and assisting them to consider community living options if their needs can be met in a less restrictive setting. The ability to work collaboratively with others, including nursing facility personnel, agency provider staff and families will be required. Services will include face to face contact with individuals on the caseload monthly. Expectations and requirements will follow the LIDDA Performance contract with HHS. General Description The purpose of this job is to screen clients for substance use issues and determine if the client meet TDI criteria for substance use treatment and DSM-5 criteria for a substance use diagnosis. Clients that are identified with co-occurring mental issues will be referred to the proper clinic. The incumbent conducts outreach activities to the community and provide drug education and information on the substance use treatment services that are available and information on the dangers of mood-altering chemicals that threaten the mental, physical and spiritual health of our community. The purpose of this job is to be a member of the Criminal Incident Response Team, which serve victims of mass casualty criminal incidents and communities throughout Readiness, Response, and Recovery phases. Specifically, Criminal Incident Response Teams serve victims of crime and communities through training, technical assistance, education, community outreach, case management, hotline support, exercises and drills, deployment and behavioral health direct care services (including therapy), assessments, and community resource linkage. Position provides counseling, crisis stabilization and support services to victims of crime that may include brief psychological interventions, therapy, Critical Incident Stress Management (CISM), or other evidence based or evidence informed best practices for victim services. The position works under general supervision, developing work methods and sequences, and is assigned to the Clinical Services Unit, and will work independently or in a team setting.
