General Description

  • This position works under the Certified Community Behavioral Health Clinic (CCBHC) Grant.
  • Responsible for leading the design, implementation, and support of a robust agency wide population health management program.
  • Identifies and recommends technology innovations and opportunities to expand and customize the reporting and data analytic platforms to meet stakeholder data needs, stratify patient populations by risk and drive client outcomes.
  • Develops analytical recommendations for holistic value-based care in a changing healthcare environment.
  • Collaborates with EHN leadership, health promotion teams, prescribers, manage care organizations and local partners in identifying and improving patient experience, costs of care and health outcomes.
  • Designs and implements a population health program to influence client care and improve health outcomes; specifically establishing the risk stratification tools and client priority listings to direct and guide the newly formed health promotion team's work.
  • Recommends point-of-care and dashboard solutions to improve agency wide performance and clinical decision making.
  • Develops strategies for program evaluation in order to demonstrate value to internal and external stakeholders, including payers and local partners.
  • Demonstrates excellent communication and presentation skills with the ability to engage and coordinate staff, community stakeholders and executive leadership.

Minimum Qualifications

Master's Degree with major specialization in a social, behavioral, or human services field of study is required. A minimum of 5 years' experience in a healthcare setting is required. Minimum of 5 years' experience in fiscal management, public policy, government agency or non-profit experience preferred. 2-5 years successful data analysis work required. Minimum of 1 years of experience with pay for performance metrics, 1115 Waiver operations, strategic planning and budgeting in large organizations.

Knowledge, Skills and Abilities

Understanding of behavioral health services best practices. Understanding of clinical and physician/patient protocols Medicaid, Medicare and other third party payment methods. Knowledge of population health strategies and pay-for-performance contracts. Ability to drive NCQA and metric performance in a large organizations. Ability to analyze an interpret data. Ability to display data to multiple audiences including board level and clinic level dashboards. Able to identify the nature of problems, develop and implement solutions, using the PDSA methodology. Tracks outcomes based on metrics and able to analyze data to improve practice operations. Strong interpersonal skills and the ability to establish and maintain effective working relationships within the organization and community. Ability to interact with diverse groups and lead them to consensus.
